Benefit Highlights Thank you for your interest in CORT Business Services. Highlights of employee benefits are outlined below. Prior to enrollment, employees receive a benefits packet that describes each of the benefits in more detail. | Medical | CORT offers you three medical plan options: ●United Healthcare POS Point of Service ●United Healthcare High Option PPO Preferred Provider Organization ●United Healthcare Low Option PPO Preferred Provider Organization Each option provides different levels of coverage, co-payments and coinsurance. All three plans allow you the freedom to choose an in-network or out-of-network provider each time you or your covered dependents need care. Additionally California resident are eligible to participate in an HMO option with Kaiser Permanente of California. | Dental | Dental coverage is separate from the medical plan election. This means you may enroll in one plan and waive coverage in the other. You may also choose to cover dependents in one plan and not to cover dependents in the other. CORT offers two dental plan options. Each option provides different levels of coverage, coinsurance, and maximum benefits. You have the freedom to choose whether to receive care from a network of MetLife Dental providers or non-network providers each time you or your covered dependents need care. | Vision | Your vision election is separate from your medical and dental plan elections. This means you may enroll in one plan and waive coverage in the other. You may also choose to cover dependents in one plan and not to cover dependents in the other. You have the freedom to choose whether to receive care from a network of providers or non-net-work providers each time you or a covered dependent need care. | LIFE INSURANCE AND ACCIDENTAL DEATH AND DISMEMBERMENT | CORT provides Basic Life and Accidental Death & Dismemberment (AD&D) insurance in the amount of $10,000 at no cost to the employee. Employees may elect to purchase additional Supplemental Life and AD&D Insurance in increments of $25,000 (ranging from $25,000 to $500,000). Employees also have the option to purchase dependent life insurance coverage and Family AD&D coverage. | LONG-TERM DISABILITY | Coverage provides 60% of your monthly base salary up to a maximum of $10,000 per month. Employees pay the full cost of this coverage through convenient payroll deduction. CORT provides a Short-Term Disability benefit to employees who elect Long Term Disability. | 401(k) INVESTMENT SAVINGS AND PROFIT SHARING PLAN | Upon completing six months of service, full-time employees and part-time employees will be enrolled in CORT's 401(k) Investment Savings and Profit Sharing Plan. 401(k) Investment Savings Plan: Employees may contribute from 1% to 60% of their earnings on a before-tax basis. Each month, CORT matches the first 4% that the employee contributes on a pre-tax basis at a rate of 50 cents per dollar. Employees may also elect to contribute on a post-tax basis. Profit Sharing: Each year, as profits permit, CORT contributes an additional match to 401(k) accounts of active participants as of December 31. | AUTHORIZED PAID LEAVE | CORT has an Authorized Paid Leave policy designed to provide full-time and part-time employees with a number of paid days off if they are sick. Details of this program will be provided upon employment. | PAID HOLIDAYS | CORT observes the six holidays below.
